NYC families: it’s time to hear from you!
Released every year, the NYC School Survey provides City families, teachers, and select support staff in public schools grades 3K-12, and students in grades 6–12, the opportunity to provide feedback about their experiences in NYC public schools. By completing this survey, you can help our schools better understand what their community members think about their learning environments, curricula, support services, and more. In fact, the information you provide can help foster conversations within your school community that will result in making your school a better place to learn!
Taking the 2021 NYC School Survey
You have two ways to complete this year’s survey:
- Complete the survey online:
You will need your child’s nine digit Student Identification (OSIS) number to complete the online survey. You can find this ID number on their report card, their student ID card, or even their NYC Schools Account. On the survey page, families must enter “f” followed by their child’s nine-digit Student Identification (OSIS) Number to access the survey. Note that “f” must be lowercase.
- Fill out a paper form
If you cannot complete the survey online, we encourage you to contact your school for assistance. Schools that requested and received printable survey forms will be in touch with families about filling them out in person.

Survey results will be shared with DOE staff members this summer to support our preparation and planning work for the 2021–2022 school year. The DOE will release the 2021 survey results in fall 2021.
